Social Distancing Booking Form

About this Demo

This form lets you book a slot for a service provided by the fictitious Penforth Council. It can be easily adapted for use by anyone who needs to implement a booking system in order to maintain social distancing in line with government guidelines. You can completely customise the data that is collected from the type of data collected as well as whether fields are mandatory or optional. You can also add a document upload field and request documents to be uploaded to prove address and customise the way in which the form is presented.

A message can be used to direct bookings to specific sites, this can be changed regularly as the need arises.
You can have multiple sites with different operating parameters, for example tip A might let 4 cars in per booking while tip B can accommodate 6. Sites can be filtered based on vehicle type.
Optionally you may supply us with your e-mail address and mobile phone number. We will use this information to send you confirmation of your booking.
